"Chronic" homelessness has been on the rise in theUSA and elsewhere. This research project focuses on agroup of "chronically" homeless street dwellers inBoston, Massachusetts in 2000-2002 who due to theirpoor health status are at high risk of death. Thisin-depth case study analyzes administrative data onhealth and substance abuse service use and associatedoutcomes. In addition, structured interviews withservice providers and street dwellers provideinformation on the process of accessing services andhousing outcomes. Research findings indicate thatonly 24 percent of street dwellers were housed at theend of 2002. Except for long respite care stays,health and substance abuse services had little impacton housing outcomes. Interview data point to servicecoordination and consistent staff support as bridgesto housing. Barriers encompass lack of affordablehousing, lack of support during transition from thestreets to housing, lack of financial supports,program eligibility rules, and personal factors.Policy implications include the need for affordablehousing, support systems for successful transitionsbetween the streets and housing, and reducingbureaucratic barriers.
